Reefing pad eye replacement?

The pad eyes that I use to tie the reeding lines to are in bad shape... One snapped a couple of weeks ago. Anyone know how to find a replacement? Preferably same screw layout? Picture attached...

I have at least one and probably two. You can have them. I took them off when I refinished the boom and mast. I also internalized the outhaul and simplified the refining system while I was at it.
